At North Eastern Ontario Family and Children’s Services (NEOFACS), we strive to provide quality service to our clients. Hearing from you, our clients, is an important way to help us make sure we are doing the best work we can.
Please access the client feedback surveys listed below by program area where you can share your thoughts on how we are doing. The surveys are open to clients currently receiving service from NEOFACS in our Child Welfare, Child and Youth Mental Health, Youth Justice, and Prevention programs.
Your survey answers are anonymous, confidential, and voluntary. We will use the feedback we receive from you and other clients to better understand what we are doing well and most importantly, how we can improve our services.

Please fill out the survey for the service you have received by clicking on the links listed below. If you have received multiple services, we encourage you to fill out a survey for each program area.

- EarlyON Child and Family Centres: Feedback can be shared online through the EarlyON Issues/Concerns Form, available through our program funder the Cochrane District Services Board (CDSB)
- Supervised Access Program (SAP) Survey
For participants in group programming of Counselling and Therapy Services (CTS), Family Preservation Program (FPP), and Brighter Futures Programs:
If you are unable to fill out the online survey or require an alternative format, please stop by any of our office locations, or contact us by phone at 1-800-665-7743 or by email at feedback@neofacs.org and we will help you to access and complete the survey.
Please do not use the feedback surveys to report a safety concern or submit a service complaint.
